Passport Appointment Request

The Waterloo Public Library, in Waterloo, Iowa, is a designated U.S. Department of State Passport Acceptance Facility. Passport services are available by appointment only during library hours, depending on staffing availability.

If you are applying for a passport, have you filled out the DS-11 form?
Do you have certified proof of U.S. citizenship?
Accepted forms are original birth certificate, U.S. Passport, or original naturalization certificate. See more information here. (If you do not have an original birth certificate, you must contact the Vital Records Office for the county in which you were born to obtain a certified copy. The library is not able to contact the Vital Records Office on anyone’s behalf.)
 
Do you have a current government-issued ID, such as a driver’s license?
Note: The U.S. Dept. of State will not accept temporary paper licenses. See acceptable forms of identification here.
 
Do you have a 2 x 2 color photo of the applicant with a white background?
(You may purchase one at Walgreens or CVS, if needed. See photo requirements here.)
 
Please note: The Library is not able to schedule passport appointments 90 minutes before closing.
I understand that the Waterloo Public Library charges a $35 fee per application that may only be paid with cash, check, or money order.
I understand the fees charged by the U.S. State Department for the passport will vary based on the options I choose, such as a passport card and/or passport book, and any additional charges I select for expedited processing and/or mailing. I also understand U.S. State Department only accepts payment in the form of a check or money order.

I understand I must bring all the required documents and payment methods to my passport appointment.
I understand that each applicant applying for a passport must be present for the duration of the appointment.
I understand that if the applicant is under 16 years of age, it is necessary for both parents or legal guardians to be present during the appointment.
(Unless the applicant can provide parental consent documentation as outlined on the U.S. State Department's website.)
 
I understand that the processing time for passport applications, including the time it takes for the application to reach the U.S. State Department through U.S. mail, undergo processing, and for the passport to be returned to the applicant via U.S. mail, may vary.
